The following lines have few trains (relative to service levels generally in this country) - fewer than three trains each way on at least three days a week - or do not run each weekday:
- Tabanovci - Preševo (Serbia)
- Veles - Kočani
- Đorče Petrov - Volkovo - Hani i Elezit/Đeneral Janković (Kosovo). Note that as at October 2017, operation of this train is extremely unreliable owing to a shortage of motive power. Prospective passengers should enquire locally as the presence of the train on departure indicators at Skopje is no guarantee that it will actually run.
